Support Coordination
NDIS plans to support those with Cerebral Palsy (CP), Down Syndrome, Autism, Muscular Dystrophy, Spinal Bifida, Acquired Brain Injury and Spinal Cord Injury often carry a significant budget and inefficient third-party management can often fritter away tens of thousands of dollars each year. This is money that could be better spent assisting clients to live rich and dynamic lives.
At Ability2Connect, we’ve lived it, with Liam taking full control of his funding after coming from the old disability system funding model many years ago. We now look to bring this experience to help our clients, either by
- working with them to transfer their plans into our care, so providing more efficient coordination, or
- teaching and supporting them and their families to take full control, moving to a self-managed model.
Ability2Connect provides a range of services to clients, but not limited to:
- Break down and explore your NDIS plan, inc budget and goals
- Teach you to navigate the MyPlace NDIS Participant Portal via myGov
- Identify your current providers, mainstream supports, health agencies and informal supports.
- Assist you to connect with other supports that you may need and meet with new provider with you.
- Assist with managing your support effectively, to ensure that you stay on track with funding and use your plan to its full potential.
- Attend NDIS planning meetings and works with Local Area Coordinators (LACs) to provide all the relevant information to ensure all aspects of, and the information relating to a client’s needs is taken into consideration.
- Ability2Connect support and guide clients in navigating services and ensuring they receive value for money.
- Work with our clients and their providers to ensure that the client’s needs and goals are being met.
